Yes, you are correct that the Record Update triggers on the field for the Checkbox Formula. Currently that formula is as such:
If(ToText([AttachmentField]) <>"", true, false)
It returns true/false as I expect it to depending on if there is anything in the attachment field, but only the first file uploaded triggers a pipeline.

I thought that "passive" changes didn't count as triggers?
I've not messed with attachment fields in a while, but can you not set the PL to trigger when the attachment field changes? You could use the checkbox field as a safe guard though. Changes to the attachment field trigger the PL, but you filter it to ONLY run if the checkbox is checked. That way, if someone deletes the attachment it doesn't run the PL.

It is also my understanding that Passive triggers do not work on these for the most part. The fact it only triggers once is what is confusing me on it. If it never worked I wouldn't have gotten in so deep on it, haha.
I actually tried it when the attachment field changes - But that doesn't seem to trigger the Pipeline at all, unfortunately. This would have made things much simpler for me.

------------------------------When you say it triggers once, is that upon uploading a file for the first time or uploading a file in a new record and then saving it?
Hmmm, I could have sworn that file uploads counted as a "active change/edit". Is your trigger step "on edit", or "On add or Edit"?

------------------------------When I say it triggers once, it is the first time an upload occurs.
Due to form rules/other issues, the first one or two times a record is saved, it's impossible to upload an attachment actually. Once that attachment is uploaded however, the pipeline will trigger, using my formula checkbox as the only trigger field.
If in the next day or so, some record is changed and hte file is re-uploaded (Mind you, it has already been deleted at this point as well) the pipeline will not retrigger, despite all of the fields being the same state (as far as I can tell).
When I delete the file, the pipeline actually tries to start up again because the checkbox field is turning back to unchecked, but I have a condition where if there isn't a name for a file (meaning it's not deleted) then the pipeline aborts early - So I know Quickbase is seeing it go back to empty. Just not sure why it won't see it come back on again!
