I'd suggest the ABC Legal Case Manager buy Kirk Trachy and then rename some of the tables to suit your use-case better. When I was fiddling with it I adjusted some of the relationships too. The screenshot below shows the relationship diagram that I would use. Really any project management app could be adjusted to suit your needs.
